
REFERRALS
In the state of Maryland, physical therapists have direct access, which means you do not need a physician’s referral to receive a physical therapy evaluation and treatment if your insurance does not require it. The exception to this rule is Medicare, which requires a script to begin care with a PT. If you have been referred by a provider, please let our staff know, so that with your consent, we can maintain communication with them during your course of care.
If you do not have a referring provider, please inquire with your physical therapist to assist you with connecting with a referring provider if necessary.
Insurance
We are happy to announce that Flourish Physical Therapy and Wellness is now in-network with Medicare, Blue Cross Blue Shield and Aetna! We are also able to process claims for Cigna if your plan includes out of network benefits. For all other insurances, we are happy to provide you with a superbill, that you may submit to your insurance company with the possibility that you will be reimbursed through your out of network benefits. We will collect payment (copay, or full session payment) at the time of service (cash, credit card, check, flexible savings accounts and health savings accounts).
Your insurance may require you to meet your deductible before your copay or coinsurance kicks in. Depending on your plan, your deductible may require you to pay the full amount charged to your insurance for each session. A copay is a set price that you are responsible to pay per session. This price may vary on the type of appointment (an evaluation may be a higher rate than a follow up). A coinsurance is a percentage of the amount billed that you are responsible to pay per session. This price will also vary based on the type of session.
A re-evaluation will be completed monthly, to assure that you are making progress to your goals. This is also required by the Maryland state practice act for physical therapists. This may also effect your copay or coinsurance rate.
If you would like a super bill, please ask at checkout and we will email an itemized bill for those interested in insurance reimbursement to independently submit to your insurance company.
CANCELLATION & MISSED APPOINTMENTS POLICY
In the event that the patient is unable to keep an appointment, please contact us as quickly as possible. We request cancellation or rescheduling 24 hours prior to visits to allow other patients to have access to that treatment time. Visits that are cancelled less than 24 hours prior to the visit time, or are not cancelled at all will be billed $70 due to scheduling inconveniences.
E-mail, phone call, or text are all suitable means to communicate visit cancellation if the message is sent 24 hours prior to the visit start time. In the case of a true medical emergency, the cancellation fee will be waived.

FAQs
What to expect your first visit
Prepare to discuss when the condition started and what your symptoms are. Please come in comfortable clothing that you can move in. You are a whole person and we will assess the specific region that is contributing to your symptoms and the joint regions that surround the affected area. If you are coming in for pelvic floor therapy, your first session may include an internal exam, though this decision is always left to the patient. We will begin treatment the same day, typically including hands on treatment (soft tissue or joint mobilization, dry needling, etc) and exercise.
What is pelvic floor PT?
Pelvic floor therapy at Flourish combines internal and external therapies in order to help you reach your fullest potential. Not every patient requires internal therapy. This is an ongoing discussion between therapist and patient. If you do choose to have an internal exam, for female patients this may be a vaginal or rectal exam, depending on your symptoms. For male patients, this will require a rectal exam. Once we determine your starting point, we will build a plan based on your specific areas that need improvement.
How often do I need to come?
Though courses of care vary, most patients start their first 4-6 weeks at a once a week pace. When the patient and therapist feel that they are progressing well, they are typically reduced to every other week, and then to once a month as their strength and home program routine have fully taken root.
What else should I know?
Your relationship with your therapist is a therapeutic alliance. We are a team. Your role is to put everything that is going on out in the open and your therapist’s job is to hear you and guide you along the right path to get you better. This requires honesty from both sides. If you feel like there is a missing link, please make your therapist aware.
